Eligibility:
The primary purpose of the experience is to offer training for the student in research methods. Student must be:
- At least 16 years of age
- Enrolled in high school, pre-baccalaureate program, community college, undergraduate program, post-baccalaureate program, or medical school
- US citizen or permanent resident
Requirements:
- Your visits may not exceed more than six (6) months
- If receiving course credit, provide proof using template. If not receiving course credit, provide letter of recommendation from your school
- A copy of your medical insurance card
- If you are under the age of 18, additional paperwork will be required (see instructions)
Instructions:
Before you start the application process:
- Identify and contact your host (supervisor)
- Identify specific dates of prospective visit
- If seeking course credit, you must have:
a. Proof of registration in the course (see templates in the Forms section)
b. Official course description
- If you are not seeking course credit, please provide a letter of support from your home institution (i.e., from a counselor, teacher, or faculty member) justifying the value of the experience.
- Please complete required documents below before you submit the initial request form. Documents will only be accepted in PDF format.
- Please have a copy of your valid medical insurance card
- Complete the initial request form

If any information is missing or incomplete, your application will not be considered. Please allow 4-6 weeks for notification that your visit has been approved or denied by the Office of Pediatric Education and the School of Medicine Human Resource Group.
